Supported Living Services
Supported Living Services (SLS) for clients with enduring special needs are centered on the value of each individual's dignity, strengths, contribution to the community and desire for decision-making and living independently whenever possible. We partner with clients and their families to design individualized service plans that allow for optimal growth and development in the least restrictive setting.
We offer a continuum of care through:
- Supported Living Apartments
- Independent Apartments
- Cluster Apartments
- Group Homes
Supported Living Apartments
Designed for individuals who are ready to transition from Group Homes or a family home into a home of their own choice. St. David's Center staff work with individuals to set up programming that support the individual to live in his/her own home. We facilitate relationships with property managers that allow for successful community living arrangements.
SLS may include flexible personalized goals in the following areas:
- Maintain and improve independent living skills
- Build relationships
- Experience community life in a way similar to their peers
- Increase health and wellness
- Implement supervision and safety
- Increase recreation and socialization opportunities
- Meet medical and financial goals
- Contribute to community
- Use assistive technology
Two options are available:
Independent Apartments
Designed for adults who can live independently with minimal support. This model is best suited for individuals that are able to be alone for periods of time throughout a 24-hour period and desire a high level of independence.
Cluster Apartments
This model is best suited for individuals that desire the flexibility or need access to on-site support staff throughout a 24-hour period. Benefits include: natural opportunities for social relationships; an option for a daily healthy, family-style meal; 24-hour support with overnight on-site supervision; ease of access to transportation and activities; minimized anxiety around transition.
Group Homes
Homes designed for individuals with the highest level of need for support. Group Homes foster growth in independent living skills while offering 24-hour live-in support.
